Map Pack Ranking Guarantees: 5 Red Flags When Hiring a Local SEO Company
October 5, 2026

If you run a membership-based practice, you’ve gotten the email. Maybe the cold call too. “We guarantee your practice #1 in the Google map pack in 30 days!” Sometimes they even attach a screenshot of another medical practice sitting pretty at the top of the map.

Here is the truth: nobody can guarantee map pack rankings. Not us, not them, not anyone. And a company that leads with that promise is telling you everything you need to know about how they will treat you as a client.

We manage local SEO for concierge practices, DPC practices, and hybrid membership models every day, physician-led, NP- and PA-led alike. We’ve also cleaned up the damage after practices hired the wrong company. Here are the five red flags we tell every practice owner to watch for.

Why the Guarantee Is Impossible

Quick refresher: Google ranks the map pack on three factors. Relevance (does your listing match the search?), distance (how close are you to the searcher?), and prominence (how trusted is your practice online?).

That middle factor is the whole ballgame. Google shows different map results depending on where the searcher is standing. Nobody, no agency, no software, controls where your prospective patients are standing when they search. A company promising you a guaranteed #1 spot is promising to control something they cannot control. That should end the conversation right there.

What a good company can promise: they will do the real work, relevance and prominence, properly and consistently. Rankings usually follow. “Usually” is the honest word. Anyone who won’t say it isn’t being honest with you.

Red Flag 1: Guaranteed Rankings (of Any Kind)

“Guaranteed #1 in the map pack.” “First page guaranteed or your money back.” These promises sound reassuring. They are the single clearest signal of a company you should not hire.

Sometimes the guarantee has fine print: they guarantee rankings for keywords nobody searches, like your own practice name plus your street address. Congratulations, you rank #1 for a search term with zero monthly searches. That is how the guarantee gets “fulfilled.”

Ask this instead: “What will you actually do each month, and how will I know it’s working?” The answer should be specific work and business metrics: calls, direction requests, profile views. If the answer is just “rankings,” keep looking.

Red Flag 2: Secret Proprietary Methods

“We use proprietary AI technology and a secret ranking system we can’t disclose.” If a company can’t explain what they’ll do in plain English, it’s because you wouldn’t like the answer.

Legitimate local SEO is boring. It’s Google Business Profile optimization, citation cleanup, a real review system, local content, and patience. There are no secrets. There are no shortcuts that Google doesn’t know about and eventually punish.

We don’t drink the Google Kool-Aid, and we don’t drink the guru Kool-Aid either. Anyone selling you a secret method is selling you the sizzle because the steak doesn’t exist.

Red Flag 3: They Want to Stuff Keywords Into Your Business Name

This one is common and genuinely dangerous. The company “optimizes” your Google Business Profile by renaming it from “Smith Family Medicine” to “Smith Family Medicine | Concierge Doctor Austin | Best Primary Care Texas.”

Keyword-stuffed business names violate Google’s guidelines. Competitors can report you, and Google can suspend your listing. A suspended listing doesn’t drop you to position four. It removes you from the map entirely while you appeal, which can take weeks. For a practice that gets most of its new patient inquiries from the map pack, that’s catastrophic.

Your business name on Google must be your real business name. Any SEO company that suggests otherwise is risking your visibility to make their reports look good.

Red Flag 4: Fake Reviews or “Review Packages”

“We’ll get you 50 five-star reviews this month!” Reviews are a real ranking factor, but purchased or fake reviews violate Google’s policies. Google removes them in waves, and businesses caught buying reviews can have their profiles penalized or labeled.

A subtler version of the same violation is the thumbs-up/thumbs-down funnel: patients who had a good experience get sent to Google, while unhappy ones get diverted to a private contact form. That is review gating too, no matter how politely it’s framed. If you want to catch unhappy patients early, do it separately: ask every patient privately how their visit went, and ask every patient for a Google review. The moment the public review ask depends on a positive private signal, you’ve crossed the line.

The legitimate version is slower and better: a system that asks real patients at the right moment. Two to four genuine new reviews a month, arriving steadily, beats fifty suspicious five-star reviews that all appeared on a Tuesday.

One more review warning while we’re here: never confirm someone was your patient when responding to reviews, even if they name themselves. A calm, professional response that doesn’t confirm the relationship protects you under HIPAA and impresses prospective patients reading along.

Red Flag 5: They Hold Your Assets Hostage

This is the one that costs practices the most money long-term. The agency builds your website on their platform, registers your domain in their name, sets up your Google Business Profile under their account, and runs your ads from their account. Everything works fine until you try to leave. Then you discover you own nothing.

Before you sign anything, confirm in writing: you own your domain, you have admin access to your Google Business Profile, you own your website and can move it, and you have full access to any ad accounts spending your money. If the company hesitates on any of these, walk away.

A related version: the company sends reports full of jargon and vanity metrics but won’t give you logins. If you can’t see what’s happening with your own eyes, you’re not a client. You’re a hostage.

What the Good Ones Look Like

Green flags are simpler than you’d think. They explain their work in plain language. They give you full access to everything. They report on business metrics, calls, bookings, direction requests, not just ranking screenshots. They tell you what they can’t do, including guaranteeing rankings. And they start with an honest assessment of where you stand before asking for a dollar.</p

That’s why we start with a free strategy call before anything else. We’ll look at your profile, reviews, and rankings and tell you straight where you stand, whether you hire us afterward or not. Schedule a call and get an honest read on your local presence. (If you’re running Google Ads, we also offer a one-time paid Google Ads audit at stevenschwartz.info.)

The map pack is the most valuable real estate in local search for a membership practice. A widely cited Moz analysis of local search behavior found 44% of searchers click a map pack listing, nearly half of all clicks going to just three businesses. It’s worth investing in properly. Just make sure the company you hire is building you an asset, not renting you a promise they can’t keep.

Been burned before, or getting pitched right now and want a second opinion? Schedule a call and we’ll tell you straight whether the pitch holds up.
